Welcome To Our Office

The doctors and staff at the Lake Zurich Foot Clinic are committed to making your first visit to our office pleasant and stress-free. To make that possible, we would like to provide you with the following information prior to your appointment:

1. When you call for your first appointment, we need to gather certain information from you. This may include basic demographics, your insurance information, and the reason you need to be seen. Please be prepared to provide us with all of this information when you call.

2. Our new patient paperwork is available on this page, to be printed off and filled out ahead of your appointment. Additionally, we can email the paperwork to you or send it via regular mail. You also have the option of coming into our office 15 minutes ahead of your appointment time to fill the paperwork. Please let us know what your preference is for completing the paperwork.

3. Please bring your photo ID, insurance card(s), medication list, referrals, and any previous diagnostic/X-ray/lab results to the appointment so we may copy this information and put it into your chart.

4. If you have any special needs, please let us know ahead of time so we can accommodate you.

5. If at any time you need assistance/information prior to being seen at our office, please do not hesitate to contact us. Our qualified and experienced staff are here to help you!
